Abstract
Accident investigation reports following fatal mistakes where flammable refrigerant ignited whilst removing a compressor, ammonia leaked during removal of a dormant compressor, and ammonia leaked into brine in an ice rink chiller; highlighting the importance of site workers having appropriate qualifications, skills, knowledge and experience.
